The COVID-19 Transmission Index is a balanced approach intended to protect communities. It represents the collaborative work of state and local public health officials, the Governor’s Office, legislative leaders, the hospital industry, and business leaders.

Data will be analyzed weekly, and counties will be placed into a transmission level depending solely on what their data show. Changes from a lower level to a higher level may occur weekly. Changes from a higher level to a lower level may occur every 14 days at minimum, when thresholds are met.

Event Management Form

All establishments that allow public gatherings, such as live events, movie theaters, sporting events, weddings, recreation, and other entertainment are required to submit an EVENT MANAGEMENT FORM for approval.

  1. Download and complete the EVENT MANAGEMENT FORM, including the signature of an authorized organizational representative responsible for ensuring event oversight
  2. Submit completed and signed forms to the Wasatch County Health Department.  You may submit forms either by email (send to he@wasatch.utah.gov), by fax (435-654-2705), or in-person
  3. A representative from the health department will review the completed form, provide feedback if changes are needed, and return the approved form  to the applicant by email
  4. Keep a copy for your records
